At the Carer Hub East, we will support you as you prepare for an NDIS planning meeting.

What does the Carer Hub offer?

  • A welcoming space
  • An opportunity to connect with other Carers
  • Support as you prepare for the NDIS
  • Information and links to other supports

We have many activities scheduled in the lead up to the Eastern roll-out of the NDIS.

All sessions are FREE. Respite can be arranged if required to attend. These sessions are open to eligible carers living in the Eastern Metro Region who provide care to a person who is living with a mental illness, intellectual disability, autism, or severe and profound disability.

Where? Whitehorse Community Centre, Level 1, 79 Mahoneys Road, Forest Hill
(Opposite Forest Hill Chase, parking available next to the building)

Plan My Future Follow-up sessions

These sessions are for carers who have already attended a lifeAssist Plan My Future workshop. These sessions give carers support to complete their workbooks, ask questions and connect with other carers. 

Carer Statement sessions

A Carer Statement is an opportunity to write a letter about your caring role to the NDIS Planner. We will support you to write your statement. 

Carer Goal Planning sessions

A Carer Goal Plan is an opportunity in an NDIS planning meeting for you, as a carer, to talk about the support that you provide to the person you care for, if the participant agrees. We will help you write your Goal Plan.


Sessions run from 9.30 to 11.30 am on the following dates:

  • Sept       Wednesday 6, 13, 20, 27
  • Oct         Wednesday 4, 11, 25,  and Tuesday 17
  • Nov        Wednesday 1, 8, 15, 29 and Tuesday 21
  • Dec        Wednesday 6, 13, 20
