lifeAssist RACGP workshop

THE aged care environment is changing and older patients with multiple chronic conditions require complex management from the GPs and Practice Managers.

lifeAssist ran a Royal Australian College of General Practitioners (RACGP) workshop for General Practitioners and practice teams informing them of the changes.

The content in the workshop was designed to assist GPs and practice teams as they support older patients and carers. Content in the workshop included:

  • discuss prevention and early intervention
  • understand the resources and supports available for your patient
  • provide a comprehensive understanding of the changes in the Aged Care environment
  • provide a range of referral pathways available to support patients

Feedback received included:
